What are the corrective and what are the aesthetic operations?
Operations treating illnesses, developmental disturbances and improving functional discrepancies of our body belong to the group of reconstructive or corrective operations. A reconstructive operation is reimbursed by the social insurance in case it is performed at a state institution.
According to the European standpoint, operations done for purely aesthetical, cosmetical purposes must be burdened by VAT. Plastic surgical interventions for the purposes of prevention or improving health state, as well as other surgical interventions, are free of VAT also in Hungary.

What do we know about the “silicone scandal” in America?
This is a strange story relating to law, which is quite far from the professional side of plastic surgery. Some very prepared lawyers brought and won precedent legal actions in the different states of America against companies manufacturing silicone prostheses. Today, professional opinions on which these legal actions were built in the past are already reckoned as very doubtful concerning their scientific value. In spite of this, in the American legal system, already won precedent legal actions are always precedent for any similar cases.
Profiting from the achievements of the computer science, practical lawyers collected a great number of American patients who had received silicone prostheses and simultaneously claimed numerous actions to recover damages by way of compensation referring to the earlier precedent legal actions. This was why the FDA (Food and Drug Administration) temporarily suspended permission to use silicone gel prostheses. However, the results of the great number of follow-up examinations of patients did not prove any mistakes of these prostheses. FDA validated again the permission to use silicone gel prostheses. In the same time, manufacturers decided not to put silicone gel prostheses into circulation till the American legal system is changed. In the same time these prostheses are still used for reconstructive operations, because for this type of operation, there have not been won any precedent legal actions. This is the reason why in America the use of saline prostheses started to spread in spite of the fact that their touching is not so natural.
Can an implanted prostheses cause breast cancer?
It can be stated surely that it cannot. In this field, the greatest number (more than 1 million) of clinical studies and statistical follow-up examinations have already been performed in the world. The results were surprising. Development of breast cancer was lower in number after the implantation of prostheses than without them. Of course, this is not a direct effect of the implanted prosthesis. It can be explained by the fact that during the examinations before the operation and during the follow-up examinations patients suffering from cancer or those susceptible to it could be more quickly and easily screened out. It is also a beneficial result of prostheses that mammography of ladies with small sized breast and less glandular substance becomes easier. Namely, the prosthesis will always be implanted behind the glandular substance. In this way the glandular substance is lifted forward to be in a better position for an examination. It has also been proved that self-examination of the patients becomes more effective as well.
What is capsular contracture?
This is a possible complication of implanted prostheses that you should know if you decide to have this operation. It is a natural reaction of our body to form a capsule surrounding any implanted foreign material (metal plates, screws, blood vessel and breast prostheses as well). This capsule against a foreign material can be contracted in the 5% of patients and can make a pressure onto the soft breast prosthesis causing its deformation or making it sensible. This means for the patient that later she might undergo another operation to treat this contracture. During this minor operation, the contracted capsule is cut open similarly to cutting the orange-peel to make enough place for the prosthesis inside. Probability of this complication and of the necessity of a second operation is 20:1, that is from 20 patients there can be 1 case when this minor operation has to be performed. Unfortunately we cannot predict who develops this complication.
Can the shape of breast be chosen?
Yes, the shape of breast can be influenced by choosing the type and position of the prosthesis. The details will always be discussed with the patient during the visit before the operation. The prosthesis will be chosen from a catalogue together with the patient. It is possible to reproduce its appearance by computer, but on the basis of my experience, that examining photos of patients with similar structure made before and after the operation is of greater help.
Is breastfeeding possible?
Yes, operations at our clinic are always done without any damages of the glandular substance. Prostheses are placed behind the glandular substance or more deeply, behind the muscles of the breast. It makes breastfeeding possible even after the operation. Silicone prosthesis is neither harmful to health of the infant. It must be noted that form of the implanted breast can change during nursing in the same way large breasts can change. Because of breastfeeding, breasts can be elongated. It cannot be protected with prostheses. According our latest experience, elongation of the breasts can be prevented with removing prostheses during breastfeeding. Later on, these prostheses can be easily implanted into their earlier place.
Is it allowed to fly after the implantation?
Yes, of course. Several of my patients flew over the Ocean one week after her operation without any observable complications. Media rumours in this field are based on imperfect theories. Within the airplanes or in cabins there is overpressure, therefore, decrease in pressure is not considerable. Decrease in air pressure can be dangerous for a prosthesis filled with water when the rate of decrease is dangerous to other tissues as well, when lack of oxygen already threatens survival.
What are the benefits and disadvantages of saline prostheses?
In prostheses filled with physiological saline have the same solution we use in infusions (thousands of litres per day). It is surely not harmful in case it comes out from the prosthesis into the body. This is their real benefit. The other benefit is that this type of prosthesis can be placed through a very short excision, because it is implanted emptily and filled through a valve after the implantation. In the same time, this valve can be a disadvantage of these prostheses. Although, the valve is very secure, it can happen that it leaks in the course of time. I grew very desperate when I had to reoperate my patient because of the defect of the valve. In my opinion, this is an unnecessary risk factor to increase the number of reoperations. Recently, they have started to manufacture fully closed prostheses without any valves filled with physiological saline. I have not yet gained much experience with them, but I am sure their touching is not so natural than that of a silicone prosthesis.
Is it suggested to use prosthesis filled with soybean oil?
Prostheses filled with soybean oil and used for clinical trials received great publicity through media after the feigned “silicone scandal” in America. However, initial hope was not confirmed by clinical experience. Results of the clinical trial expressly show that soybean oil will damage the wall of the prostheses during some years and it can run out of them. Soybean oil will form an insoluble “soap compound” in our body. Today, professionals all over the world have an expressed standpoint that prostheses filled with soybean oil must not be implanted. Fortunately, Hungarian plastic surgeons – very thoughtfully – have never used this type of prostheses.
Which is the best prosthesis?
There is no homogeneous standpoint between plastic surgeons in the same way as we want to establish which is the best car. Professionals cannot have the same opinion, but this is not a problem. We choose according to two points of view: safety and results that can be achieved. During aesthetic operations of the breasts, my primary goals (besides my patients’ safety) are to reach natural breast form and natural touching. To reach these goals I think the silicone prostheses are the best. Within silicone prostheses and with operational techniques used by myself, my experiences are the best with cohesive gel prostheses. There are only a few companies manufacturing these products, but all of them are available in Hungary. In these prostheses, gel is like meat-jelly. They keep their form, they do not have even any molecular outflow, and they will neither flow when it is cut in two. They are safe and their touching can hardly be distinguished from touching natural glandular substance of the breasts.
Their only disadvantage is that they are more expensive than the simple silicone prostheses. They can be reached for those who do not choose Trabant when they want to buy a car.
What other methods exist to enlarge breasts?
Extracts of medicinal herbs
There are many kinds of preparations with different prices available in the shops. I have not heard about any durable effects caused by any of them. They can be seriously dangerous if some hormone preparations are mixed into the natural herbs. Unfortunately, in our country, natural preparations not designated as medicaments are often not thoroughly and strictly studied. Material and moral background necessary for these examinations are not always reliable.
With these preparations enlargement of breasts can be reached beyond question. But we must not forget what penalty we have to pay for it! Preparations containing significant amount of hormone act not only on the breasts but on the inner female genital organs. Their prolonged administration surely causes significant side effects which will manifest later. Act of hormonal preparations on the size of breasts lasts only until the preparation is taken, moreover, the act can be stopped earlier, because our organism can get used to higher hormone levels, and hormone receptors of the breast cannot be swindled for a longer period. From hormone preparations I can recommend only contraceptives to be taken under control. In the same time, this act can be discussed with the gynaecologist.
Results that can be reached with hypnosis are very doubtful. Generally, it is not the physicians who advertise this method. In certain parts of the world, occult sciences are more popular, in spite of this, I have already seen advertisements in Budapest as well: “enlargement of breasts with laying on of hands”. What should a professional say about it? Perhaps we can say that it does not have any effects but it is at least a good feeling. And what is more, it is not harmful to health!
Training of muscles of the breasts
Muscles of the breast can be strengthened with physical exercises which can have be favourable concerning the form and size of the breasts as well. However, it can have its most favourable effect on improvement of posture. But be careful, because it can have an adversary effect in case it is not applied appropriately. Exaggerated strengthening of the breast muscles can lead to decreasing fat content that influences the size of the breasts.
This is in experimental stage to enlarge breasts. Its clinical application is not yet spread generally. Breasts are placed underneath a capsule with rigid wall and the breasts are continuously aspirated in order that cells of their connective tissue could proliferate. Applying these awkward capsules continuously for a period of 3-6 months, the size of breasts could be enlarged successfully with 10-05%. I predict success of this system in spite of the fact that its application is very uncomfortable, and its price will be similar to that of the implantation. We do not know whether this system will be chosen in the future instead of breast prostheses. I am sure it will not. Moreover, it will make plastic surgery more popular because those who are so determined to have their breast correction performed, they want real results. However, real results can only be expected from plastic surgery.
